Veterinary Records Release Authorization

When you authorize us in the Omi app, we generate a signed records release addressed to the veterinary practice you named, and we send it to them with our request for your dog’s file. This page is the text of what you are authorizing. It is written for you; the document the practice receives says the same things in the form a practice expects.
This authorization sits alongside our Terms of Service and Privacy Policy, and Section 10 of the Terms covers records requests and electronic signatures generally. Where those documents and this one overlap, this one governs the records release itself.
Granting it is optional. Nothing about your membership, your testing, or your results depends on it. Records make your dog’s baseline richer, because we start from what is already known rather than from nothing — but declining costs you no part of the Service.
1. What you are authorizing
You authorize Omi Health Inc. and its authorized representatives to request, receive, and review the complete veterinary medical records for the dog you named, from each veterinary practice you named.
You authorize each of those practices to release those records to us, and you ask them to treat this authorization as your written direction to do so.
You are naming specific practices. An authorization covers only the practices listed in it, and only the dog listed in it. You can name up to three practices at a time, and you can grant a further authorization later for another practice, another dog, or to correct a name you got wrong.
2. Which records this covers
Everything the practice holds for your dog at the time we ask, including:
- examinations, visit notes, and clinical history;
- diagnoses and assessments;
- treatments, procedures, and surgical reports;
- laboratory and diagnostic test results;
- imaging studies and reports;
- vaccination and parasite-prevention history;
- prescriptions and medication history;
- any other health information currently on file.
Practices sometimes include client information in a patient file — your address, phone number, or your payment history with them. Where they do, that information comes to us with the records.
3. What we tell the practice about you
So the practice can identify the right patient and confirm the request is genuine, the authorization we send contains:
- you — your name as it appears on your Omi account, the email address on that account, and your phone number and mailing address where we hold them;
- your dog — name, and breed and date of birth where you have given them to us;
- the practice — the name, city, and state you entered, and the phone number or email address if you gave us one;
- the authorization — the date you granted it, the date it expires, a reference number, and a statement that it was granted electronically through the Omi app.
We take these details from your account rather than from the request itself, so what the practice receives matches the account the authorization attaches to.
4. What we do with the records
We use records obtained under this authorization to coordinate and improve care for the dog named in it: adding their history to that dog’s record in the app, informing the guidance the app produces, and giving the veterinarians and specialists involved in your dog’s care the context to answer well.
Records obtained under this authorization are not shared with third parties, except as needed to provide the service you asked for — the providers that host and process data for us, under written terms, as described in Section 9 of our Privacy Policy.
We do not sell your dog’s records. We do not license or rent them. They are never sent to insurers, employers, breeders, registries, or data brokers in identifiable form.
5. How long it lasts
90 days from the day you grant it, unless you withdraw it sooner.
After 90 days the authorization is spent. We do not treat it as renewing, and if we need to ask a practice again after that we will ask you for a new one.
Expiry does not require us to delete records we already hold — those stay in your dog’s record in the app, and are governed by the retention and deletion terms in our Privacy Policy. You can ask us to delete them at any time.
6. Withdrawing it
Email hello@omihealth.com at any time and we will withdraw it. You do not need to give a reason, and withdrawal has no effect on any other part of your membership.
Two limits, and we would rather be straight about them:
- Withdrawal works going forward. It stops us making further requests under that authorization. It cannot recall records a practice has already sent us — though you can ask us to delete those, and we will.
- It cannot undo a release already made. If a practice acted on the authorization before we told them it was withdrawn, that release already happened.
7. Redisclosure
Once records are released to us, the information may no longer be protected by the veterinary confidentiality rules that applied at the practice, and may be subject to redisclosure by the recipient. This is the standard consequence of any records release, and it is why Sections 4 and 6 matter: what protects those records once they reach us is our own commitments and your ability to withdraw and to have data deleted.
8. Release of the practice
You release each veterinary practice you named, and its veterinarians, staff, and agents, from any liability arising from the good-faith release of records under this authorization.
This is the assurance a practice needs before releasing a file to a third party, and it is limited to exactly that: releasing records in good faith, to us, because you asked them to. It releases nobody from anything else, and it releases Omi from nothing at all.
9. What you are confirming
By authorizing, you confirm that:
- you are the owner of the dog named, or otherwise legally authorized to make decisions about them and to direct the release of their records;
- you are the client of record at each practice you named, or otherwise authorized to release records held there;
- the details you gave us — the dog, and each practice’s name and location — are accurate to the best of your knowledge;
- you are at least 18 years old and able to enter into this authorization.

10. Your electronic signature
You grant this authorization by authorizing it in the Omi app while signed in to your account. That action is your signature. There is no typed name and no separate form to sign.
Under the federal Electronic Signatures in Global and National Commerce Act (15 U.S.C. § 7001 et seq.) and the Uniform Electronic Transactions Act as adopted in your state, an electronic authorization has the same legal force and effect as a handwritten signature. You consented to transact with us electronically under Section 10 of our Terms of Service, and this authorization relies on that consent.
11. What we record as proof of consent
At the moment you authorize, we record:
- the account that granted it, and the email address on it;
- the date and time, taken from our servers rather than from your device;
- the network address and browser or app details of the request;
- a reference number for each authorization, printed on the document the practice receives;
- a copy of what the authorization said — your details, your dog’s details, and the practice named — frozen as they were at that moment, so the record does not change if you later edit your profile.
We keep this for as long as we need it to evidence the authorization. It exists to establish that a real, signed-in account holder gave a specific consent at a specific time. We do not use it for advertising or analytics.
12. Getting a copy
Email hello@omihealth.com and we will send you a copy of any authorization you granted, including the document sent to the practice. Ask at any time, including after it has expired.
We suggest saving the copy. The app shows you which practices you have authorized us to contact, but it does not host the document itself.
13. What this authorization is not
- It is not consent to treatment. It authorizes a records request, nothing more. Omi does not diagnose, prescribe, or treat — Section 1 of our Terms of Service sets out those limits.
- It does not make Omi your dog’s veterinary practice, and it creates no veterinarian-client-patient relationship with your existing practice or with us.
- It does not authorize us to send anything to your practice about your dog beyond the request itself. Sharing an Omi report with your vet is your decision to make, separately.
- It does not oblige a practice to respond. Some practices are slow, some charge a copying fee, and some decline. We will tell you if a request goes unanswered.
- It is not a consent to marketing, and it does not change your email preferences.
14. Changes to this document
If we change how records requests work, we will update this page and the effective date at the top. Changes apply to authorizations granted after they take effect — an authorization you have already granted is governed by the version in force when you granted it, and we keep the earlier versions so we can tell you which one that was.
